Minsky's - Booking Form

1. Make sure the date below is for the performance you wish to see.
2. Enter the type of ticket and the number of tickets you require.
   IMPORTANT
   For ALL tickets except STANDING ONLY tickets, you need a Booking Reference for you to continue.  Please call Minsky's on 02920 233128 if you do not have a reference number.
3. If you wish the tickets to be posted out to you click the checkbox, otherwise you can pick up your tickets any day before the performance during our normal daytime opening hours.
4. Once all your details have been added press the "Print" button to print off this form then enclose it with your cheque and post to: Minsky's Showbar, 42 Charles Street, Cardiff, CF10 2GE.
